If a person loses a credential by shipwreck or other casualty that causes damage to a ship, a duplicate will be issued free of charge as per 46 CFR 10.229. If a person loses a credential by other means and applies for a duplicate, the appropriate fee set out in 46 CFR 10.219 must be paid. No application from an alien for a duplicate credential will be accepted unless the alien complies with the requirements of 46 CFR 10.229.

5.MMC Endorsement(s) - This is a statement on a mariner's MMC that indicates that he or she is qualified to serve in that capacity. All endorsements including National officer and National rating endorsements as well as all STCW endorsements (International) are listed in 46 CFR 10.109.

6.Document of Continuity - This is a record of qualifications previously held and does not authorize the holder to sail in any capacity listed thereon. Documents of continuity do not expire, do not require medical or security evaluations, and do not require fees. STCW endorsements may not be placed in continuity. At the time of application you must provide a written disclosure of all prior convictions NOT previously disclosed.

Original Applicants are required to list ALL convictions.

Written Disclosures - Applicants may use the optional form (CG-719C) to provide written disclosure of all convictions.

If an applicant pleads guilty or no contest, is granted deferred adjudication, or is required by the court to attend classes, make contributions of time or money, receive treatment, submit to any manner of probation or supervision, or forgo appeal of a trial court's conviction, then the Coast Guard will consider the applicant to have received a conviction. A later expungement of the conviction will not negate a conviction unless the Coast Guard is satisfied that the expungement is based upon a showing that the court's earlier conviction was in error.

III.3 National Driver Registry (NDR):

No MMC will be issued as an original or reissued with a new expiration date, and no new officer endorsement will be issued if the applicant fails the criminal record review in accordance with 46 CFR 10.213.

What You Should Know About This Form

What is the purpose of the CG-719B form?

The CG-719B form is used to apply for a Merchant Mariner Credential (MMC), which is required for individuals seeking to work on U.S. vessels. This includes applying for original credentials, renewing existing ones, obtaining duplicates, or seeking new endorsements. Additionally, it serves those who need a medical certificate.

Who needs to fill out the CG-719B form?

Anyone applying for an MMC must complete this form. This applies to first-time applicants, those renewing credentials, individuals requesting duplicates due to loss, and those wanting to raise their grade or add endorsements. It is essential for both U.S. citizens and legal aliens to provide the necessary information.

What information is required on the CG-719B form?

The form requires personal details such as your legal name, Social Security number, date of birth, citizenship status, and address. You’ll also need to disclose any relevant criminal history and provide contact information for an emergency contact. This comprehensive information ensures proper vetting for safety and suitability for maritime work.

How do I ensure my application is not delayed?

To avoid delays, it is crucial to submit a complete application. Double-check that all required sections are filled out and that all mandatory documents accompany your application. Keep your contact information updated, as the National Maritime Center (NMC) may need to reach you for any follow-ups.

What happens if my application is incomplete?

If your application is incomplete, the Coast Guard may refuse to process it. This could lead to significant delays in obtaining your MMC. To minimize the risk of an incomplete application, refer to the detailed instructions provided with the form and ensure all documents are attached.

How can I check the status of my application?

After submitting your application, you can check its status by contacting the National Maritime Center at 1-888-IASKNMC (1-888-427-5662) or visiting their website. Providing an email address on your application may also allow you to receive automated updates regarding your application status.

Common mistakes

Completing the CG-719B form can be a straightforward process if all requirements are met. However, many applicants make common mistakes that can lead to delays or denials in obtaining their Merchant Mariner Credential (MMC). One prevalent issue is failing to provide a complete legal name. This section requires your full name, including any aliases or maiden names. If an applicant lists an incomplete or incorrect name, it raises verification concerns, which could delay processing.

Another frequent oversight is neglecting to include required identification numbers. Specifically, applicants often forget to provide their Social Security Number (SSN) or alien registration number if applicable. This information is crucial for the Coast Guard to verify identity and eligibility. An empty or inaccurate field can halt the application process, possibly causing a significant setback for the applicant.

Inaccuracies in the applicant's date of birth or place of birth frequently create complications as well. An incorrect date can suggest that the applicant is underage or fail to match records in government databases. Place of birth should also be entered precisely. For individuals born outside the U.S., leaving the state blank is necessary, and not adhering to this guideline can result in confusion during the verification process.

Contact information errors represent a critical mistake that may lead to communication issues. Providing a PO Box instead of a street address is a common error. The Coast Guard requires a residential address because they need to send important documents through the mail. Additionally, omitting a primary phone number or email address minimizes the chances of being contacted regarding application status. Missing this information increases the likelihood of application delays.

Lastly, failing to sign and date the application is a crucial mistake that can lead to rejection of the entire form. The form requires the applicant's signature to verify that all provided information is accurate and truthful. If this section is neglected, the Coast Guard will return the application without processing it, causing unnecessary delays. Reviewing and double-checking each part of the form can help avoid these common pitfalls.

Documents used along the form

When applying for the Merchant Mariner Credential (MMC) using the CG 719B form, you may need to gather additional documents to support your application. Each document plays a unique role in ensuring your application is complete and compliant. Here’s a list of essential forms and documents that you might find beneficial during the application process.

  • CG-719C: This optional form is used for disclosing any criminal convictions. It ensures that the U.S. Coast Guard can conduct a thorough review of an applicant’s history regarding safety and suitability.
  • TWIC (Transportation Worker Identification Credential): This credential is crucial for accessing secure areas of vessels and facilities, as required by the Maritime Transportation Security Act. You must have either a TWIC or proof that you have applied for one.
  • Medical Certificate: Applicants must prove they meet the medical and physical health requirements to safely operate a vessel. A medical examination report is typically needed.
  • Proof of Citizenship: Documents such as a birth certificate or passport are vital to establish your citizenship status. Non-citizens may require additional identification, such as an Alien Registration Number.
  • Sea Service Records: These are documents verifying your maritime experience. They detail the time spent on vessels, roles performed, and type of vessels sailed, all necessary for the MMC application.
  • Document of Continuity: While this won't authorize sailing, it records previously held qualifications and can facilitate the application process for certain categories.
  • Entry Level Ratings Application: If you are applying for entry-level positions, this application substantiates your qualifications for roles like Ordinary Seaman. Medical verification by a physician is often required for health-related positions.
  • Third-Party Authorization Form: If you want someone else to act on your behalf regarding your application, this written consent allows specified individuals or organizations to discuss your credential status with the Coast Guard.
  • Criminal Record Checks: Certain criminal history disclosures may be required beyond the CG-719C. This may involve consent for background checks from relevant authorities.

By gathering these documents ahead of time, you can streamline the application process for your Merchant Mariner Credential. Each piece of paperwork is a crucial step towards ensuring that your application is successfully processed, getting you one step closer to your maritime career.

Dos and Don'ts

Things to Do When Filling Out the CG 719B Form:

  • Double-check your legal name. Make sure it matches official documents and includes aliases if applicable.
  • Ensure your Social Security Number is correct if this is your original application.

  • Attach a notarized consent form if you are under 18 years old, as this is mandatory.
  • Sign and date the application. A missing signature will result in delayed processing.

Things to Avoid When Filling Out the CG 719B Form:

  • Do not use a PO Box as your home address. It is not acceptable.
  • Avoid skipping sections. Each part of the application must be filled out completely.
  • Do not provide false information. This can lead to denial of your application.
  • Never ignore the requirement for a digital signature or fail to include it.
  • Don't forget to include your contact information. This will help the NMC reach you without delays.

Misconceptions

  • Misconception 1: The CG-719B form is only needed for new applicants.
  • Many believe that only first-time applicants for a Merchant Mariner Credential (MMC) need to fill out the CG-719B form. In reality, this form is also required for renewals, duplicates, and upgrades, making it essential for anyone maintaining or seeking to update their credentials.

  • Misconception 2: Providing your Social Security Number (SSN) is optional.
  • Some applicants think they can skip the SSN field on the application. However, if you are applying for an original MMC, your SSN is mandatory. This information is used to verify your identity and ensures the proper processing of your application.

  • Misconception 3: You can use a P.O. Box as your home address.
  • There exists a common belief that P.O. Boxes can be used for home addresses on the CG-719B form. Contrary to this assumption, the Coast Guard requires a physical residence address. Failing to provide the appropriate address could lead to delays in your application process.

  • Misconception 4: A suspended license will not affect my MMC application.
  • Many applicants underestimate the impact of a suspended driver’s license on their MMC applications. If you have ever had your driver's license revoked or suspended for serious infractions, you must disclose this on the form. The Coast Guard conducts a thorough review of your criminal background, which includes this information.

  • Misconception 5: The CG-719B form can be submitted without any supporting documents.
  • Some individuals might believe it is acceptable to submit the CG-719B form without accompanying documents. However, the Coast Guard emphasizes that any application must contain all necessary supporting documentation. Incomplete submissions can result in delays or even denial of the application.

Key takeaways

The CG-719B form is essential for anyone seeking a Merchant Mariner Credential (MMC). Here are some key takeaways to keep in mind when filling out and using this form:

  • Eligibility: Before submitting the CG-719B form, ensure that you meet the eligibility criteria. This includes being a U.S. citizen or a legal alien and fulfilling any medical requirements.
  • Accurate Information: Provide complete and accurate information. This includes your full legal name, Social Security Number, place of birth, and contact details. Incomplete applications may be refused for processing.
  • Documentation: Attach all required documents. Depending on your situation—such as if you are applying for an original MMC or a renewal—you may need different supporting documents. Ensure your application is accompanied by the necessary paperwork.
  • Criminal Record Disclosure: Be prepared to disclose any criminal convictions. This includes any drug use or violations of maritime law. Transparency is crucial, as the Coast Guard reviews your criminal record during the application process.
  • Timely Submission: Submit your application promptly. Delays can occur if the Coast Guard cannot reach you, or if you fail to comply with the required processes. Keep an eye on deadlines, particularly for renewals.
