What is the RST Army form used for?
The RST Army form is a Request for Authorization for Rescheduled Training (RST), Equivalent Training (ET), or an Excuse from Regular Scheduled Unit Training. Who is responsible for completing the RST Army form?
The applicant, which can be the service member themselves along with their Flight Line Leader (FLL) and Platoon Sergeant (PSG), must complete the relevant sections of the form. Necessary information such as name, unit assignment, dates of absence, and reasons for absence must be printed or typed accurately.
What information must be provided in Part I of the form?
Part I requires various details including the applicant's name, unit assignment, DMOS, rank, Social Security Number (SSN), dates of absence, and the number of Unit Training Assemblies (UTAs) missed. What happens after the form is submitted?

Their decision must be communicated back to the service member, and if approved, the soldier is required to perform the designated duties and submit a DA Form 1380 along with a copy of the approval to the Orderly Room.
